Balance Pilates Policies
In order to provide our clients with clear communication and fair service, we ask that you read the following carefully.
1. Appointment Cancellation Policy
Balance Pilates has a 24-hour cancellation policy. This means that if an appointment is not cancelled within the cancellation policy time, the student will be charged for the session or class.
If the student feels that extenuating circumstances have occurred, he/she should speak to the director to make different arrangements.
2. Package Expiration Policy
Pilates Personal Training packages expire 6 months from date of purchase. Medical exceptions, only. If you know you will be going on vacation, or are Seasonal, please discuss with the director at time of purchase.
3. Duet and Small Group Personal Training Policy
Duets: If part of a duet cannot attend a lesson, the partner has the choice to (a) cancel the lesson or
(b) to pay an additional $20 for a private lesson.
Trios: If one or two partners cannot attend a lesson, the partner(s) have the choice to (a) cancel the lesson or
(b) to pay an additional $20 for a duet training session, or $30 for a private lesson.
Quads: If one or more partners cannot attend a lesson, the partner(s) have the choice to (a) cancel the lesson or
(b) to pay an additional $15 for a trio lesson, $20 for a duet session, and $35 for a private
4. Personal Training Refund Policy
If a physician has determined that there are medical reasons that you cannot complete your package, we will refund the unused portion of your package, minus a $25 administrative fee. A written explanation from your physician is required in order to process your refund request.
There are no exceptions to this.
5. No Refunds on Gift Certificates
There are no refunds for gift certificate purchases. You may transfer unused gift certificates to another person, gift it as a charitable donation, or transfer unused portions to another service at Balance Pilates. No exceptions.
6. Liability Waiver and Informed Consent Release Form
All students must sign a Liability Waiver and Informed Consent Release form,
which is printed out below for your information.
I have enrolled in a program of instruction in a method of physical conditioning offered by Balance Pilates & Yoga Center. I have been advised and I understand that participation in Balance’s exercise and conditioning activities, like any physical conditioning activity or exercise program, presents some unavoidable risk of injury, especially to people who have pre-existing injuries, illness, or medical disabilities. I understand that the use of exercise equipment also carries with it a risk of injury. I hereby affirm that I am in good physical condition and do not suffer from any disability that would/could prevent or limit my participation in this exercise program.
In consideration of my participation in Balance’s conditioning program, I, for myself, my heirs, next of kin, guardians, executors, administrators and assigns, hereby release Balance Pilates and Yoga LLC, its owners, officers, directors, shareholders, employees, trainees, and contractors from any claims, demands and causes of action arising from my participation in the conditioning program.
I fully understand that I may injure myself as a result of my participation in Balance’s conditioning program, and I hereby release Balance Pilates and Yoga LLC from any liability now or in the future, including but not limited to heart attacks, muscle strains, pulls, or tears, broken bones, shin splints, heat prostration, knee, lower back or foot injuries, and any other illness, soreness or injury, however caused, occurring during or after my participation in the conditioning program.
I also understand that a medical evaluation is advisable before commencing any program of physical conditioning or exercise. I have and will continue to keep Balance fully informed of any physical condition or disability which would prevent or limit my participation in an exercise or physical conditioning program. I consent to the use of tactile cues to direct the proper body positioning of an exercise. Balance Pilates and Yoga Center shall not be responsible or liable for any articles lost, stolen or damaged, in or about the studio or on location.
